4. Orange – substitute!

We had a diabetic patient who was constantly relapsed with extremely high sugar levels. We asked him if he was following the treatment we taught him… checking his blood sugar, using a sliding scale, measuring the correct insulin dose in the needle, etc.

We asked him to describe the steps he took to observe and correct any mistakes he might have made. He did everything correctly until the last grade. He drew insulin from a syringe, pulled an orange from his pocket, then injected insulin into the orange and ate it. We finally found the cause.
